Sauna and Stroke Risk: What the Research Shows

Stroke is among the leading causes of death and long-term disability worldwide, so anything that plausibly lowers the risk deserves a close look. Over the past decade a run of studies from Finland has repeatedly linked regular sauna bathing with better cardiovascular outcomes, and one of the more striking findings concerns stroke specifically.
The headline comes from a 2018 prospective cohort study: people who took a sauna four to seven times a week had a substantially lower risk of stroke than those who went once a week. This article looks at what that study actually measured, why heat exposure might protect the brain's blood supply, and how much weight the finding can reasonably bear.
One caveat up front. Almost all of this evidence is observational and drawn from Finnish populations, where sauna bathing is a near-universal habit. Association is not causation, and frequent sauna users may differ from infrequent ones in ways that independently affect stroke risk. The numbers below are best read as encouraging signals rather than guarantees.
What the stroke study found
The central evidence comes from the Kuopio Ischaemic Heart Disease study, which tracked 1,628 men and women aged 53 to 74 with no history of stroke at baseline. Over a median follow-up of roughly 15 years, 155 first strokes occurred. Compared with participants who bathed once a week, those who used the sauna four to seven times a week had a hazard ratio for stroke of 0.39, a 61% lower risk, while two to three sessions a week carried a smaller reduction of around 14% 1.
What makes the result harder to dismiss is that it survived statistical adjustment. After accounting for conventional risk factors including blood pressure, type 2 diabetes, smoking, physical activity, socioeconomic status and cholesterol, more frequent bathing remained independently associated with lower stroke risk, and there was a graded trend across the three frequency groups 1. The effect was seen in both men and women, which matters because much of the earlier Finnish sauna research had studied men only.
Why heat might protect the brain's blood supply
Stroke and high blood pressure are tightly linked; hypertension is the single most important modifiable risk factor for stroke. This is where the sauna's most consistent physiological effect becomes relevant. In a separate analysis of the same broad Finnish cohort, men who took a sauna four to seven times a week had a 47% lower risk of developing hypertension over a median of nearly 25 years than those bathing once a week 2.
The acute physiology points in the same direction. In an experimental study, a single 30-minute sauna session lowered systolic and diastolic blood pressure and improved a measure of arterial stiffness (carotid-femoral pulse wave velocity), with the effect persisting into the recovery period 3. Sitting in the heat dilates blood vessels, raises heart rate and places a cardiovascular load that has been compared to moderate exercise, and repeated exposure appears to nudge the vascular system towards greater compliance.
A review of the evidence proposes several overlapping pathways: improved endothelial function, reduced systemic inflammation and oxidative stress, better regulation of the autonomic nervous system, and favourable changes in blood pressure and arterial stiffness 4. Any of these could plausibly reduce the risk of both the clot-driven (ischaemic) and bleed-driven (haemorrhagic) forms of stroke, though the review is careful to note that the mechanisms remain under active study.
Where this fits in the wider cardiovascular picture
The stroke finding does not sit in isolation. An earlier study from the same research group followed 2,315 middle-aged men for a median of around 20 years and found that higher sauna frequency was associated with lower rates of sudden cardiac death, fatal coronary heart disease, fatal cardiovascular disease and all-cause mortality. Men bathing four to seven times a week had roughly half the risk of fatal cardiovascular disease of those going once a week 5. Read together, these cohorts describe a consistent gradient: across several cardiovascular endpoints, more frequent bathing tracks with better outcomes.

How much should you read into it
It is worth being disciplined about the limits. These are observational cohort studies, not randomised trials, so they cannot prove that the sauna itself caused the lower stroke rates. It is possible that people healthy enough to bathe often, or with the leisure and means to do so, were already at lower risk for reasons the statistics could not fully capture. The evidence is also concentrated in Finland, where sauna culture, session style and genetics may not generalise neatly to everyone 4.
The sensible reading is that regular sauna bathing is a promising, well-tolerated habit that lines up with a broad pattern of cardiovascular benefit, but it is not a substitute for the fundamentals of stroke prevention: keeping blood pressure in a healthy range, not smoking, staying physically active, and following the advice of a clinician if you have existing cardiovascular disease. Anyone with low blood pressure, unstable heart disease or a recent cardiovascular event should speak to a doctor before making heavy sauna use a routine.
